Updated vaccine schedule 2017

·         Effective July 1, 2018 ALL students in kindergarten through twelfth grade must show proof of having received two doses of Hepatitis A vaccine to attend school.

·         Students aged 16 years or older must show proof of having received two doses of Meningococcal ACWY vaccine (MenACWY) to attend school.   If the first dose of MenACWY was received at age sixteen (16) years or older, the second dose is not required for school entry.

IN ADDITION: The KY Immunization Form has been updated. ALL students in the district will be required to submit to their school a new Immunization certificate- Updated Immunization Certificate (even if their immunizations are up to date). Any student who has a religious exemption will be required to submit a new religious exemption form that now requires a notary seal.

Parents can call their Dr and request their immuization record be Transfered to the New KY Immunization Form. The Dr can then fax the form to the school (East High Fax- 881-0161); or when school is out of session, to Pat Gass' office, fax mumber (887-4993)
